We provide comprehensive care for Medicare patients.

We invite Medicare patients and their families to take advantage of our team-oriented, multidisciplinary approach.

Gwinnett Clinic Provides Medicare Services to Patients With:

IMPORTANT: We do not see patients on Medicare Advantage plans at this time.

  • We care for you like you are a part of the family. You will truly get to know your team of compassionate doctors, nurse practitioners, and staff. Relationships are the cornerstone of keeping you healthy!
  • Annual Wellness Visit: You will have an annual review of your health. Most insurance plans cover this cost.
  • Access Seven Days Per Week: Your trusted team is available for all of your chronic problems and sick visits. No need to go to the emergency room or urgent care - unless it is a true emergency!
  • Same-Day Sick Visits: As soon as you experience the onset of an illness, don't wait to be seen. Either call  ahead or walk-in to be seen by a member of your team.
  • Easy Access for Patients with Mobility Challlenges: This is particularly helpful for patients who have trouble walking. No parking decks or elevators at most of our primary care offices!
  • Home Delivery of Medications: We offer a free service coordinated by our in-house pharmacy.
  • Convenient Preventive Services: Scheduled at one facility. Check our Services page for details.
  • Balance & Gait Training: Led by our in-house physical therapist with support from our orthopedics and neurology teams.
  • Memory Loss & Dementia Care: Managed by our in-house neurology team.
  • Advance Care: End-of-Life Planning.

Medicare Chronic Care Management

What is Chronic Care Management (CCM)?

Chronic Care Management is a Medicare program that provides monthly care coordination for patients with multiple chronic conditions.

WHO IS ELIGIBLE FOR THE CCM PROGRAM?

Any Medicare patient with two or more chronic conditions is eligible. Some of the most common chronic conditions are:

  • Arthritis
  • Alzheimer’s
  • COPD
  • Diabetes
  • High blood pressure (hypertension)
  • High cholesterol
  • Heart failure
  • Kidney disease

WHAT WILL CCM INVOLVE?

As a CCM patient, your primary care provider will spend extra time coordinating your care on a monthly basis. This could involve working with a pharmacist to manage your medications or communicating with other specialists you see. You will also receive a phone call once a month from a Gwinnett Clinic staff member to check on your conditions.

WHAT IS A COMPREHENSIVE CARE PLAN?

Your provider will work with you to create a plan to manage your chronic conditions. This plan will include:

  • A list of all diagnoses
  • Measurable treatment goals
  • Symptom management
  • Medication management
  • A plan for coordinating care from multiple specialists

WHAT IS REQUIRED TO PARTICIPATE?

Depending on your insurance, Chronic Care Management may require a monthly copay up to $9. While you participate in CCM at Gwinnett Clinic, you will not be able to enroll in CCM at any other organization.
