Helpful tips for your next office visit. Secure & convenient access to your personal health information via our online portal!

Thank you for taking the time to visit us online. If you have any updates to share with us regarding your address, insurance, or other status, please contact us by calling your appointment location.

Helpful Tips For Your Visit to Gwinnett Clinic:

  • To schedule an appointment, please call one of our 17 locations.
  • Bring a valid photo ID and your insurance card / information.
  • Co-pays and office visit fees are expected to be paid at the time of each visit.
  • If your insurance coverage has changed, please bring your updated insurance information.
  • If you do not have insurance, a physician or nurse practitioner may still be able to see you. Call ahead to find out about affordable rates for you.
  • All new and established patients fill out medical forms prior to their appointment.

Log in to your Patient Portal

Through our free patient portal you can:

  • View lab or radiology results – Your lab results will be available on your portal within 5 business days of your visit. We will still call you to discuss and review your results!
  • Check parts of your medical record.
  • Upload documents for your provider team to review.
  • Communicate with your provider teams.
  • Check your referrals.

Register for the Patient Portal

If you would like to register for your portal today or are having issues registering, logging in, or resetting your password, please email [email protected] with the following information:

  1. First and last name
  2. Year of birth
  3. The email address you would like connected with your portal to receive notifications
  4. Reason for email: Portal registration or portal issue
